Job Description: We are seeking a dedicated and compassionate Community Nurse / District Nurse to join our team at Bromley Healthcare. As a Community Nurse / District Nurse, you will provide high-quality nursing care to patients in their homes and community settings. You will work closely with other healthcare professionals to ensure comprehensive and coordinated care.
Key Responsibilities:
- Delivering nursing care to patients in their homes and community settings
- Assessing, planning, implementing, and evaluating patient care plans
- Administering medications and treatments as prescribed
- Providing health education and support to patients and their families
- Collaborating with multidisciplinary teams to ensure holistic care
- Maintaining accurate and up-to-date patient records
- Participating in clinical audits and quality improvement initiatives
Requirements:
- Registered Nurse (RN) with valid NMC registration
- Previous experience as a Community Nurse or District Nurse is preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
