Our Client, St Helens Council, is seeking an Adults Social Worker to join their Integrated Discharge Team.

Responsibilities of this role:

  • The role will entail working with individuals, families, carers, and communities to enable them to make informed decisions, to promote independence, whilst working in a strengths-based approach.
  • Assessing patients and other tasks associated with facilitating hospital discharge, to support patients with the support they require to return home or to be discharged to another setting.
  • Undertaking preventative work and risk assessments utilising community resources and services such as reablement and telehealth care is also a key part of the role.
  • You will construct value-for-money care packages, and commission and arrange appropriate services.
  • You will be responsible for the completion of Care Act assessments, capacity assessments and Court work - section 21a challenges.

Requirements of this role:

  • Must be a Qualified Social Worker, registered with Social Work England.
  • Must have a Social Work qualification (Diploma in social work, social work degree or equivalent)
  • Must have minimum of 3 years experience post qualifying working as a Qualified social worker
