DescriptionSt?Helens Council is seeking an experienced and motivated Social Worker to join our Transitions to Adulthood Team within the Children with Disability Service. ?You will play a pivotal role in ensuring that young people with disabilities experience a smooth, well-planned move from children’s to adult services, promoting independence and positive life outcomes.Responsibilities
  • Work with young people (14?–?25) who have disabilities or complex needs, creating person-centred transition plans.
  • Coordinate multi-agency input from health, education, adult social care and the voluntary sector.
  • Complete Mental Capacity Act assessments, including Best-Interest decisions, and contribute to DoLS and Court of Protection applications where required.
  • Provide advice and support to families, ensuring they understand options, rights and pathways into adult services.
  • Maintain accurate, timely case records and prepare reports for panels, reviews and legal processes.
  • Champion a strengths-based approach that maximises independence, education, employment and community participation for each young person.
Requirements
  • Qualified Social Worker (BA / MA / DipSW) with Social Work England registration.
  • Experience in children’s or adult disability services, ideally within transition planning.
  • Working knowledge of the Care Act 2014, Children & Families Act 2014, Mental Capacity Act 2005, DoLS and Court of Protection processes.
  • Proven ability to form positive working relationships with young people, families and multi-agency partners.
  • Strong assessment, planning, report-writing and organisational skills.
  • Commitment to safeguarding, equality and promoting the voice of young people.
