About the Role:
St Helens is seeking a dedicated Social Worker to join their dynamic Hospital Discharge Team. This role offers the opportunity to make a meaningful impact on patients' lives by ensuring smooth transitions from hospital to home.
What will your responsibilities be?
In this role, you will carry out assessments and related tasks to facilitate timely hospital discharge for patients. You will support individuals to return home or transition to appropriate discharge settings with the necessary care arrangements in place. You will operate effectively in a fast-paced, high-pressure environment, ideally with prior experience in discharge planning. This is a full-time position (37 hours per week), involving a combination of office-based, ward-based, and remote working on a rota. You will collaborate closely with health and social care professionals to ensure safe, person-centred discharge outcomes.

Essential Requirements:
- Social Work England registration
- Eligibility to work in the UK
- Full UK driving license
- Post-qualified experience
- Degree level or equivalent in Social Work
