About the Role:

Liquid Personnel is recruiting a Qualified Social Worker for the Adult Hospital Discharge Team at St Helens. To be successful in this role you must have registration with the Social Work England (SWE) as a Social Worker, holding a qualification equivalent to the Diploma in Social Work (DipSW).

What will your responsibilities be?

In this role, you will complete assessments and related tasks to support hospital discharge for patients at Whiston Hospital. You will facilitate patient transitions to home or other care settings, ensuring appropriate support is in place. Operating effectively in a high-pressure, fast-paced environment, with experience in discharge planning preferred, you will fulfil a full-time role (37 hours per week), working between 8:45 am and 5:15 pm, with a mix of office, remote, and ward-based work. Additionally, you will collaborate with multidisciplinary teams to ensure safe, person-centred discharge outcomes.
