Job Title: Band 6 Physiotherapist - Discharge to Assess (D2A) Location: Musgrove Park Hospital - Primary Care & Neighbourhoods Service Area: Community Services - GDJ68 Band: 6 £25.48 Contract Type: Full Time (Part Time considered) Start Date: ASAP Vacancy Status: Unfilled Pay Rate: Agenda for Change - Band 6 (Allied Health Professionals PayScale) - £25.48 Umbrella Equivalent Role Overview: We are seeking a skilled and motivated Band 6 Physiotherapist to join the Discharge to Assess (D2A) service based within the Community Services team at Musgrove Park Hospital. This role supports patients who have been recently discharged from hospital, enabling safe recovery and reablement within their own homes. The post holder will work collaboratively with D2A-trained provider agencies to assess patients, deliver targeted rehabilitation, and set realistic SMART goals to maximise independence and support long-term care planning. Key Responsibilities: Deliver community-based physiotherapy to adults post-discharge Carry out functional assessments within the patients home environment Set patient-led SMART goals and monitor progress toward independence Work closely with reablement care providers to facilitate seamless support Identify and recommend ongoing support needs or referrals where appropriate Contribute to timely and effective discharge planning and recovery pathways Maintain accurate documentation and communicate effectively within the MDT Essential Requirements: HCPC-registered Physiotherapist Recent community experience within adult reablement services Strong knowledge of rehabilitation principles and outcome-based planning Full UK driving licence and access to own car (essential for cross-county visits) Ability to work independently and manage a caseload across various locations Comfortable working in a fast-paced, dynamic discharge service Desirable: Experience in Discharge to Assess (D2A) or hospital-at-home models Familiarity with local community care structures and health/social care integration Additional Notes: The role is part of a dynamic service designed to support short-term reablement following hospital admission. Flexibility is essential, as while the service is organised by geographical clusters, cross-county travel may be required to meet patient needs. A full-time candidate is preferred, though part-time availability can be discussed.
