Job Title: Social Care Practitioner

Location: Shropshire Council
Rate: 14.91 PAYE / 19.61 Umbrella
Hours: 37 hours per week (Mon-Thurs 8:45-5pm / Fri 8:45-4pm)
Contract: 3 months (potential for extension)

Role Overview:

Opus people solutions are looking for someone to join the Adult Social Care team to support the delivery of person-centred services aligned with the Care Act 2014, the Council's values, and an early intervention and prevention model.

You'll work with adults including older people, individuals with physical or learning disabilities, sensory impairments, and their carers-helping them achieve independence through assessments, planning, and access to services.

Desirable - Driving Licence and access to own vehicle.

Key Responsibilities:

  • Deliver assessments and support plans in line with the Care Act 2014
  • Promote early intervention, prevention, and strengths-based approaches
  • Support timely hospital discharges
  • Work collaboratively with professionals to assess need and coordinate care
  • Ensure accurate records, attend meetings, and represent the service
  • Promote equality, anti-discriminatory practice, and health & safety standards
  • Participate in rotas (e.g., emergency/same-day responses)

Essential Skills & Experience:

  • Experience working with older adults, individuals with disabilities, and carers
  • Knowledge of the Care Act 2014, personal budgets, and self-directed support
  • Understanding of reablement, assistive technology, and person-centred models
  • Ability to work across professional teams and advocate for service users
  • Familiarity with the needs of informal carers and community-based support
