Wrexham County Borough Council is seeking a dedicated Social Worker to join their vital team, focusing on the seamless transition and ongoing support for individuals with mental health needs.

Pay Rate: £46.62 per hour

About the Role:

As a Social Worker specialising in Mental Health and Hospital Discharge, you will play a crucial role in assessing, planning, and coordinating safe and timely discharges for individuals from hospital settings, ensuring they receive the appropriate support within the community. Your responsibilities will typically include:

  • Conducting comprehensive assessments of individuals' mental health, social care needs, and capacity, adhering to relevant legislation (e.g., Mental Health Act, Mental Capacity Act, Social Services and Well-being (Wales) Act).
  • Developing robust discharge plans in collaboration with service users, families, carers, and multi-disciplinary teams (MDT) including health professionals.
  • Identifying and coordinating access to suitable community-based services and ongoing support to promote recovery and prevent readmission.
  • Managing complex cases, including those involving safeguarding concerns for vulnerable adults.
  • Acting as a key point of contact for service users and their families during the hospital stay and transition to community care.
  • Maintaining accurate and timely records, and contributing to team meetings and reviews.
What We're Looking For:
  • A Social Work Degree or equivalent relevant degree.
  • At least 2 years of post-qualifying experience working with adults in mental health services, with significant experience in hospital discharge planning, in the UK.
  • Registered with Social Care Wales.
  • An up-to-date DBS (Disclosure and Barring Service).
