POSITION TITLE: CASE MANAGER

DEPARTMENT (S): CASE MANAGEMENT

REPORTS TO: DIRECTOR OF CASE MANAGEMENT

EXEMPT/NON-EXEMPT: Non-exempt

POSITION SUMMARY:

Under the general supervision of the Director of Case Management, the Case Manager is responsible for assisting in the implementation of the Case Management Program to ensure ongoing assessment of patient care delivery according to established standards.

1. Current licensure in Texas as a Registered Nurse or Licensed Vocational Nurse.
2. Experience in health care delivery systems with focus on Case Management or Performance Improvement preferred.
