Legacy Care is seeking a dedicated and experienced Nurse Practitioner to join our team on a full‑time basis in Charlottesville, VA. The successful clinician will deliver comprehensive primary and acute care services to residents in post‑acute and long‑term care settings, with an emphasis on maintaining function, managing complex chronic conditions and promoting a person‑centered approach. Key Responsibilities — Nurse Practitioner, Post‑Acute Care
Conduct comprehensive assessments and develop individualized care plans with measurable goals in collaboration with residents, families and facility staff
Diagnose and manage acute and chronic medical conditions commonly seen in post‑acute and long‑term care populations
Provide medication management, including reconciliation, monitoring for effectiveness and adverse effects, and deprescribing when appropriate
Coordinate transitions of care such as admissions, transfers and discharges, communicating effectively with hospitals, families and community providers
Participate in care‑planning meetings, interdisciplinary rounds and family conferences to ensure holistic, person‑centred care
Provide urgent on‑site assessment and management of clinical changes, initiating interventions and escalating care when necessary
Document clinical encounters accurately and promptly in the Electronic Medical Record (EMR)
Support quality and safety initiatives including infection prevention, falls reduction and medication safety
Provide clinical education and consultation to nursing staff and facility teams on chronic disease management, wound care, palliative approaches and best practices
Maintain professional standards of practice and ensure compliance with regulatory and facility requirements
Essential Qualifications and Skills — Nurse Practitioner, Charlottesville VA
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution
Current and unrestricted Virginia Nurse Practitioner licence and APRN certification
Clinical experience in geriatrics, long‑term care, post‑acute care or comparable settings preferred
Proficiency with Electronic Medical Record systems and clinical documentation
Strong clinical judgement, interpersonal and communication skills; ability to work collaboratively within multidisciplinary teams
Commitment to trauma‑informed, person‑centred care, cultural sensitivity and ethical practice
Current Basic Life Support (BLS) certification required; additional certifications relevant to geriatric or acute care preferred
