We are recruiting for the following position:

Sea Mar is seeking a Care Coordinator to support patients with chronic conditions and behavioral health needs. As a key member of the Clinical Care Team, you will work at point of care to identify patient needs, close care gaps, and coordinate services that improve patient outcomes and quality of care.

Key Responsibilities

  • Participate in daily huddles and identify gaps in care
  • Provide brief interventions and support chronic disease management
  • Coordinate care transitions and follow-up after ER/hospital visits
  • Link patients to behavioral health, dental, and community resources
  • Track and document patient progress in the EHR
  • Support quality improvement initiatives and clinical performance measures
  • Assist with 340B medication assistance programs (site-dependent)

Qualifications

  • Medical Assistant training + 1+ year experience in community health or equivalent
  • Strong patient engagement, communication, and problem-solving skills
  • Experience with chronic disease and behavioral health preferred
  • Bilingual English/Spanish strongly preferred
  • Must obtain CPR within 90 days of hire
  • Valid WA driver’s license and proof of insurance required
  • COVID-19 vaccination required
