Key Responsibilities
- Provide nursing care within the scope of your licensure and clinical competencies
- Maintain up-to-date, accurate clinical records for each patient, meeting all documentation deadlines
- Continuously evaluate the effectiveness of nursing care for patients and families
- Consult with physicians regarding changes in patient care plans and document all updates
- Communicate changes in patient condition to the nurse supervisor
- Support FFHC’s mission by mentoring and/or coaching new employees during patient-specific orientations
- Educate patients and caregivers on disease processes, treatments, and home care needs
- Participate in FFHC’s Quality Assurance and Performance Improvement (QAPI) program as requested
- Actively seek opportunities to improve clinical skills and knowledge
- Attend and/or complete all mandatory trainings and in-services
- Submit required personnel file documentation in a timely manner
- Adhere to FFHC compliance and ethics expectations
- Maintain patient and family confidentiality at all times
- Demonstrate professional, supportive, and responsive communication skills
- Perform other related duties as assigned
Qualifications
Required
- LPN/LVN Nursing Degree
- Unencumbered LPN/LVN license in the state of employment, or multi-state license through the Enhanced Nurse Licensure Compact (where applicable)
- Minimum 1 year of nursing experience
- Valid driver’s license with proof of insurance (in applicable states)
- Ability to pass all required background screenings
- Working knowledge of state Nurse Practice Acts
- Valid and current CPR certification with in-person, instructor-led skills check
- Clear Hepatitis B screening (in applicable states)
- Clear TB screening (in applicable states)
- Ability to read and communicate in English, both verbally and in writing
Preferred
- Previous pediatric or home health experience
- Comfort working independently in a home setting
State-Specific Requirements
Florida:
- Follows all general requirements listed above
Illinois:
- Valid driver’s license with proof of insurance: Not required (staff are prohibited from driving patients)
- Hep B screening is not required prior to hire and is completed during onboarding
Pennsylvania:
- Additional background checks: Child Abuse and FBI
- TB screening: 2-step PPD, or 1-step PPD with proof of prior 2-step PPD within the last year
Texas:
- Follows all general requirements listed above
Working Conditions & Physical Requirements
- Care is provided in the patient’s home; training may occur in an office setting
- Not substantially exposed to adverse environmental conditions
- Must be able to lift up to 35 pounds without an assisted lifting device
- Frequent use of office equipment, including telephone, computer, and printer
Schedule & Travel
- This role requires travel to patients’ homes to deliver care
- Shifts may vary (8-, 10-, or 12-hour shifts)
- May require availability any day of the week (Sunday–Saturday)

The typical starting pay range for this role is influenced by multiple factors, including:

- Education and certifications
- Geographic location
The full pay range reflects the competitive labor market for this role across the national market and allows for progression as team members grow and develop. Some positions at Family First Homecare may also include a bonus component, with details defined in the role-specific compensation plan.
