Job Summary
The Outpatient Care Coordinator serves as a key connection point between the Outpatient clinical team and Scheduling departments. This role supports provider caseload management, schedule optimization, and continuity of care to ensure clients experience seamless service delivery.
The Care Coordinator collaborates closely with Scheduling, Health Information Management (HIM), Compliance, Quality Improvement (QI), Medicaid Personnel, and Support Services to maintain regulatory compliance, maximize provider productivity, and support efficient caseload transitions. This position plays a vital operational role in strengthening care coordination and preventing service disruptions.

Essential Job Functions
Scheduling & Caseload Management
Serve as liaison between the OP department and Scheduling teams to ensure provider schedules are maximized.
Assist in “scrubbing” schedules in collaboration with Administrative Supervisors and providers.
Monitor caseload sizes, waitlists, and cancellation lists to promote efficiency and reduce service gaps.
Collaborate with Scheduling when providers terminate or resign to ensure smooth caseload transfers and avoid client disruption.
Work with Centralized Scheduling teams to identify incorrect client-provider pairings and coordinate necessary transfers.
Care Coordination & Interdepartmental Collaboration
Serve as liaison between OP and HIM for transfers, discharges, and caseload management.
Collaborate with OP, HIM, Support Services, ROI, QI, and Medicaid personnel to ensure client charts are managed in compliance with regulatory standards.
Assist in maintaining documentation compliance and adherence to best practice standards in partnership with HIM, Compliance, and QI teams.
Abide by privacy and security policies when interacting with clients and staff at all times.
Administrative & Organizational Responsibilities
Function in accordance with all organizational policies and procedures.
Participate in required meetings and collaborative initiatives.
Engage in regular performance evaluations of self (and assigned staff, if applicable).
Perform additional duties and special assignments as directed by supervisor.
Knowledge, Skills, and Abilities
Language & Communication Skills
Ability to read and interpret safety rules, procedural manuals, and operational instructions.
Ability to write routine reports and correspondence clearly and professionally.
Strong communication skills when interacting with providers, scheduling teams, administrative staff, and community contacts.
Mathematical Skills
Ability to perform basic mathematical functions including addition, subtraction, multiplication, and division using whole numbers, fractions, and decimals.
Reasoning Ability
Ability to apply logic and understanding to carry out written, verbal, or diagram-based instructions.
Ability to manage multiple concrete variables within standardized systems and workflows.
Additional Skills
Ability to establish effective working relationships via phone and in person.
Ability to follow direction and accept supervision.
Proficiency in PC skills and electronic systems.
Strong organizational and time-management abilities.
Ability to maintain confidentiality and professional boundaries.
Experience and Education Qualifications
Bachelor’s Degree required (Psychology preferred).
Licensure is not required; however, experience and knowledge related to clinical services is strongly preferred.

Employment Requirements
Successful completion of background checks (criminal record, driving record, abuse/neglect, fingerprint).
Completion of New Hire Orientation at start of employment.
Completion of all required training (including Relias) at hire and annually thereafter.
Current driver’s license, acceptable driving record, and active auto insurance.
Physical Requirements
ADA Consideration – Sedentary Work
Exert up to 10 pounds occasionally; negligible force frequently.
Repetitive hand and arm movements for typing and documentation.
Primarily seated work with occasional standing or brief walking.
