Job Description:
POSITION SUMMARY
The Ambulatory Care clinical pharmacist provides direct patient care within an ambulatory setting,
practicing under approved Collaborative Practice Agreements (CPAs). This role focuses on
comprehensive medication management, chronic disease state management, medication access
optimization, and population health initiatives. The pharmacist collaborates closely with primary care
providers and the interdisciplinary care team to initiate, modify, or discontinue medication therapy in
accordance with evidence-based protocols, with the goal of improving clinical outcomes, medication
safety, patient experience, and cost-effective care. The position also contributes to quality assurance
activities and continuous improvement efforts to advance ambulatory care services.
POSITION QUALIFICATIONS
Education/Licensure/Certification:
·Current California Board of Pharmacy – Licensed Pharmacist required.
·Doctor of Pharmacy (PharmD) required.
- Post-Graduate Year 1 (PGY1) Pharmacy required.
- Post-Graduate Year 2 (PGY2) Pharmacy Residency preferred.
- Board Certification (BCPS, BCACP) preferred.
- Basic Life Support (BLS) Certification required.
Knowledge/Experience:
·Experience working in an ambulatory care or primary care clinic setting providing direct patient care required.
·Knowledge of ambulatory pharmacotherapy and clinical decision-making, including evaluation of laboratory data and medication use.
·Experience identifying medication-related problems and recommending evidence-based interventions to optimize therapy.
Skills:
·Skill in reviewing patient medical records and developing individualized medication management plans.
·Ability to communicate patient care plans, clinical recommendations, and medication-related concerns effectively to physicians and other healthcare professionals.
