The Post-Acute Network Manager provides daily oversight to VCU Health’s Post-Acute Care (PAC) continuum, its formally contracted Post-Acute Care Continuing Care Network (PAC-CCN), and related programs and strategies including PAC Funding Support programs for indigent patients. This position is part of the VCUHS Continuum Integration Center and works daily with a variety of clinical and non-clinical departments across the health system and beyond.  The position coordinates and oversees performance improvement initiatives as it relates to PAC strategies and patient transitions of care across the care continuum, to ensure care is provided in a high-quality, cost-efficient and person-centric manner.  The role offers a well-balanced, hybrid work design: 3 days in-office and 2 days remote work per week.



The Post‑Acute Network Manager oversees a broad portfolio of responsibilities, including:

Managing relationships with skilled nursing facilities, home health agencies, inpatient rehab, LTACHs, and other post‑acute partners
Monitoring network performance, quality metrics, readmission rates, and throughput
Supporting discharge planning workflows and reducing avoidable delays
Leading initiatives that improve patient outcomes, length of stay, and care transitions
Serving as a liaison between acute care, post‑acute partners, and system leadership


The successful candidate will have:

5+ years related health care experience
Knowledge of post-acute healthcare, human services, and social service programs
Strong organizational, communication, problem-solving, and empathy skills
Experience with performance measurement, outcome analysis, monitoring tools

This position is aligned closely with VCUHS Continuum Integration Services and the Department of Care Coordination and will work daily with a variety of clinical and non-clinical departments across the health system.

The Post-Acute Network Manager provides daily oversight to VCU Health’s Post-Acute Care (PAC) continuum, its formally affiliated Post-Acute Care Continuing Care Network (PAC-CCN), and related programs and strategies including PAC Funding Support programs for indigent patients. This position coordinates and oversees performance improvement initiatives as it relates to PAC strategies and patient transitions of care across the care continuum, to ensure care is provided in a high-quality, cost-efficient and person-centric manner.

The Post-Acute Network Manager is responsible for oversight and monitoring of the PAC network’s performance as it relates to risk-based initiatives, such as bundled payments, ACO patients, and other Alternative Payment Models.

Licensure, Certification, or Registration Requirements for Hire: N/A

Licensure, Certification, or Registration Requirements for continued employment:
Active Professional Healthcare license (RN/OT/PT/ST/MSW) as applicable (preferred)

Experience REQUIRED:
Five (5) years of related experience in acute care hospital and/or post-acute care required.
Experience/knowledge of moving patients from acute to post-acute settings.
Clinical knowledge and experience, particularly with the senior population.
Strong/effective problem solving skills.
Experience with performance measurement, outcome analysis, monitoring tools.
Excellent computer and communication skills (oral and written)
Strong organizational skills and leadership skills

Experience PREFERRED
Project management and/or performance improvement experience preferred (PDSA, Lean, Six Sigma, etc).
Previous experience in staff education and professional development (orientation, preceptor ship, in-services education, etc.)
Previous experience in an academic healthcare setting

Previous experience in Value Based Care models, managed care, and/or provider relations.

Education/training REQUIRED:
Bachelors Degree required.

Education/training PREFERRED:
Baccalaureate Degree in health administration, business, therapy, nursing, social work, or other healthcare related field from an accredited program.

Masters Degree in Health Administration or Business (preferred)


Independent action(s) required:
Leads and manages complex projects that are strategic in nature and scope
Completes projects timely and within budget
Tracks project and subproject timelines and milestones
Self-manages daily schedule, exhibiting autonomy and superior prioritization skills

Supervisory responsibilities (if applicable): May supervise assigned subordinates.

Additional position requirements:
Schedules, including requirements of shift rotations and hours of work may be adjusted as necessary to meet program requirements and/or needs.
50% or more travel required primarily within Richmond and the surrounding counties.
