Clinical Ambulatory Pharmacist

CentraCare is seeking a Clinical Ambulatory Pharmacist to join our Medication Therapy Management (MTM) team and help improve the health of rural communities.

In this role, you will provide comprehensive medication management services to ensure each patient’s medication therapy is indicated, safe, effective, and convenient. You’ll work directly with patients while collaborating with an interprofessional care team to optimize medication use and improve health outcomes.

This position is ideal for a pharmacist who brings strong clinical expertise and values building positive, trusting relationships with both patients and colleagues across the care continuum.

Schedule

• Full-time | 80 hours per two weeks | Onsite
• Days | Monday-Friday | Primary hours 8am-5pm 
• No weekends or holidays! 

Pay and Benefits

• Starting pay begins at $124,033.47 per year and increases with experience
• Salary range:  $124,033.47- $186,422.61 per year
  • Salary range is based on a 1.0 FTE, reduced FTE will result in a prorated offer rate

Qualifications

• PharmD degree and/or BS Pharm degree required.
  • Completion of a PGY1 or PGY2 ambulatory care residency or comparable experience in ambulatory clinical pharmacy highly preferred
• Current licensure by the Minnesota State Board of Pharmacy or eligibility for licensure required. 
• Eligible for Pharmacy Specialty Board Certification preferred
• Basic Life Support (BLS) Certification
