PRIMARY RESPONSIBILITY:
The Behavioral Health Clinician works in conjunction with the Interdisciplinary Team (IDT) at On Lok PACE at the Institute on Aging (IOA) to identify and implement behavioral health services for PACE participants.

The Behavioral Health Clinician collaborates as a member of the participant's care team to identify clinical strategies for serving individuals with behavioral health needs, and to ensure that behavioral treatment is integrated into the interdisciplinary, person-centered care plan.

SPECIFIC RESPONSIBILITIES:

  • Conducts comprehensive behavioral health assessments and treatment planning, including diagnosis, participant-directed goals, and treatment recommendations.
  • Delivers and documents behavioral health interventions, including symptom management, psychotherapy, and crisis intervention support.
  • Facilitates individual and group therapies tailored to the needs and goals of the participant population and their family members.
  • Collaborates with the Medical Social Worker (MSW), Primary Care Provider (PCP) and other IDT disciplines as needed to monitor the efficacy of behavioral health interventions.
  • Collaborates with psychiatrists and Primary Care Providers (PCPs) to monitor participants' responses to psychotropic medications.
  • Educates and guides the IDT on managing behavioral complexities.
  • Documents all service delivery and encounters in the participant's electronic health record (EHR) according to program and department standards.
  • Educates participants and their support system about available resources to support their needs, and offers psychosocial education to encourage self-management of needs.  
  • Attends and actively participates in IDT and program meetings, activities and problem-solving endeavors; contributes to open lines of communication within the team.
  • All other reasonably related responsibilities as assigned.

 
REQUIREMENTS: 

  • Doctorate in Psychology (PhD or PsyD) required.
  • Current and unrestricted licensure in the State of CA as a Clinical Psychologist (PsyD).
  • Minimum of five (5) years of behavioral health experience providing therapy and completing behavioral health assessments.
  • At least one year of professional experience working with the frail elderly population.

 
BACKGROUND AND EXPERIENCE: 

  • Experience working in a team-based model of care highly preferred.
  • Experience with and understanding of the medical and psychosocial problems of functionally impaired adults and the frail elderly.
  • Bilingual in English/Cantonese highly preferred.

COMPENSATION:

Range: $ 130,000.00 - 148,000/annual

This amount is not necessarily reflective of actual compensation that may be earned, nor a promise of any specific pay for any specific employee, which is always dependent on actual experience, education and other factors.
