Key Responsibilities:
- Provide expert support and guidance to children with disabilities and their families.
- Conduct comprehensive assessments to determine individual needs and develop tailored support plans.
- Collaborate with multidisciplinary teams to ensure holistic care and support for children and families.
- Advocate for the rights and well-being of children with disabilities within the community.
- Maintain accurate records and documentation in accordance with professional standards and regulatory requirements.
Requirements:
- Qualified Social Worker Registered with SCW.
- Proven experience in supporting children with disabilities in a social work capacity.
- Strong communication and interpersonal skills, with the ability to build trusting relationships with children and families.
- Excellent assessment and case management skills.
- Ability to work effectively within a multidisciplinary team and collaborate with external stakeholders.
- Commitment to upholding the highest standards of professionalism, ethics, and safeguarding.
