As a Support Coordinator, you’ll be at the heart of supporting our customers who have complex mental health needs to develop their skills to live more independently in the community. Typical day as a Support Coordinator:

  • Creating support plans with your customers and coordinating with the service manager and mental health clinical practice lead.
  • Customer group - Mental Health, Learning Difficulties, Autism, Substance Misuse
  • Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation.
  • Carrying out risk assessments, support planning, goal setting and regular reviews and liaising with Community Mental Health Teams, Adult Social Care and other key partners.
  • Cuppas, chats and catch ups. There’s no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You bring

  • Passion to support our customers to live their best life, working collaboratively with an eye for detail. 
  • Experience of creating person-centred support plans and working towards them.
  • The ability to work on your own initiative, remain calm under pressure and have a resilient approach
  • Due to limits on our current certificates of sponsorship, we are not able to offer sponsorship to?external?candidates for this role. This remains under regular review

Job details

  • Part time permanent up to 17.5 hrs per week
  • Monday to Sunday, 2/3 sleep in shifts per week on a rolling 2 week rota. Shift times are 5pm-10pm + sleep + 8am-10am the following day. Sleep paid separately.
  • Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
  • You’ll need an Enhanced DBS check done and we pay for that.
