As a Social Worker specialising in Continuing Health Care, you will be responsible for conducting face-to-face assessments in the London Borough of Barnet. This pivotal role demands at least three visits per week to effectively assess and support individuals in need. Reporting to the Lead Practitioner, you will collaborate closely with colleagues to ensure comprehensive and compassionate care is provided.

Key Responsibilities

  • Conduct face-to-face assessments with individuals requiring Continuing Health Care within the London Borough of Barnet.
  • Work collaboratively with individuals, their families, and other healthcare professionals to develop personalised care plans.
  • Provide ongoing support and advocacy to ensure individuals receive the appropriate services and support they require.
  • Maintain accurate and detailed records of assessments, interventions, and outcomes.
  • Stay updated on relevant policies, procedures, and legislation related to Continuing Health Care.

Requirements

  • Qualified Social Worker with relevant experience and registration with the appropriate regulatory body.
  • Proven experience in conducting CHC assessments is essential.
  • Strong understanding of the CHC process and relevant legislation.
