Social Worker

AKTON Recruitment is working closely with the local authorities and we are looking for an Social Worker in the Barnet area. 3 - 6 months contract position.

Job Role:

  • To provide a high standard of social work to users arising out of the Council's duties and powers under legislation and in accordance with divisional and area policy, procedures, guidelines and the Code of Practice for Social Care Workers.
  • To facilitate a smooth transition for young people with a disability and/or learning disability and autism from into adults services.
  • Operating within a multidisciplinary environment to provide appropriate, professional Social Work support for vulnerable young people, adults with physical and sensory impairment, learning disability, and autism and carers with a focus on meeting the social care outcomes as articulated in the Department of Health's Adult Social Care Outcomes Framework.
  • Within the context of an enablement approach1 and operating within an integrated service, to provide individualised, outcomes-led, personalised support, based on structured, individual assessments that inform the setting up and commissioning of appropriate enablement packages of support.
  • Undertake the more complex assessments, packages of care and long term support for people who are eligible for services that require a professional/ specialist input.


Role-specific responsibilities
* To conduct or support individual assessments or facilitate self-assessments using the appropriate assessment tools and techniques in order to establish clear, relevant, proportionate, cost effective and appropriate options for the individual.
* Based on individual structured assessments and in line with the enablement approach, plan and set up appropriate personalised and outcome-led packages of support that maximise long-term independence and choice and minimise ongoing support and whole life cost of care for that person.
* Working with SEND and health services participate in Education, Health and Care Plan process and reviews.
* Conduct risk assessments with adults and carers, in line with eligibility criteria and Mental Capacity legislation.
* To conduct Mental Capacity Act assessments in line with statutory guidance and ensuring adults are provided with the opportunity to demonstrate capacity wherever possible.

Qualifications

  • Registered as Social Worker with the General Social Care Council
  • CQSW/ DipSW or equivalent qualification
  • Training in relevant legislation appropriate to client group
  • Training in safeguarding of vulnerable adults

Experience

  • Relevant experience of operating effectively as a practicing Social Worker with a similar client group.
  • Understanding and experience of operating with a comprehensive assessment framework including selfassessment.
  • Experience of preparing clear and accurate correspondence, reports and other written documentation.
  • Understanding and experience of working with vulnerable clients within a care environment
  • Record of high performance in successfully delivering a customer focused service to demanding targets and objectives.
  • Experience of actively and effectively participating in cross-functional networks and local groups.
