Role Overview:

We are seeking an experienced Hospital Social Worker specialising in Discharge to Assess, Care Act, and Continuing Health Care to join our dynamic team. Based in Barrow, Cumbria, you will play a pivotal role in ensuring safe and timely discharges from both acute and community hospitals, thus reducing the risk of delayed transfers of care. Our primary focus is on prevention interventions, working collaboratively with stakeholders to support individuals using a strengths-based approach.

Responsibilities:

  • Conduct assessments and facilitate the discharge planning process for patients in acute and community hospitals.
  • Collaborate with multidisciplinary teams to develop comprehensive discharge plans that address the individual needs of patients.
  • Utilise knowledge of the Care Act and Continuing Health Care to ensure appropriate support services are in place post-discharge.
  • Work closely with third sector and voluntary groups in Cumbria to coordinate short-term interventions aimed at supporting individuals in the community.
  • Employ a strengths-based approach to empower individuals and promote independence in their care journey.
  • Provide advocacy and support to patients and their families throughout the discharge process.

Requirements:

  • Qualified Social Worker with significant experience in discharge planning within a hospital setting.
  • In-depth knowledge of the Care Act and Continuing Health Care.
  • Strong assessment skills and ability to develop personalised care plans.
  • Excellent communication and interpersonal skills to engage with patients, families, and multidisciplinary teams.
  • Ability to work collaboratively with stakeholders from diverse backgrounds.
  • Flexibility to work primarily from Furness General Hospital and Craven House Barrow in Furness, with the possibility of one day remote work per week.
  • Must reside close to the base as accommodation cannot be provided.

Requirements:

  • Registered with Social work England
  • A Degree in Social work or Diploma in SW, CQSW, CSS
  • Right to Work
  • Enhanced DBS check, dated within the last 12 months
