If so, we want to hear from you!Reed Health and Care are recruiting on behalf of a leading home care provider in Liverpool, offering a range of services to support people to live independently and safely in their own homes. We are looking for a care coordinator to join our friendly and professional team and help us to provide the best possible care to our clients.

As a care co-ordinator, you will be responsible for:• Managing and co-ordinating the care packages of our clients, ensuring that they receive the right care at the right time from the right carer• Liaising with clients, families, carers, health and social care professionals and other agencies to ensure effective communication and collaboration• Monitoring and reviewing the quality and outcomes of the care delivered, and identifying and resolving any issues or concerns• Supervising care staff, and ensuring that they comply with the policies and procedures of the organisation and the relevant regulations and standards• Maintaining accurate and up-to-date records and reports using our electronic care management system• Participating in the on-call rota and providing support and guidance to care staff and clients as neededTo be successful in this role, you will need:• Preferred to have NVQ Level 3 in Health and Social Care or equivalent qualification• At least 1 years previous experience as a care coordinator or similar role in a home care setting• Excellent communication and interpersonal skills, both verbal and written• Ability to work under pressure and prioritise tasks• Ability to work flexibly and adapt to changing situations• Good IT skills and proficiency in using care management software• A full UK driving licence and access to a vehicleBenefits:• Competitive salary of 26k p.a. and bonus scheme• 28 days annual leave (including bank holidays)• Pension scheme• Mileage allowance• Ongoing training and development opportunities• Career progression and support• Friendly and supportive work environmentIf you are interested in this opportunity, please apply online with your CV or email with any enquires.
