We are looking for customer focused individuals, ideally with some primary care experience to work as a Care Navigator for our client in Bracknell. As a Care Navigator you will be answering telephone calls from patients to reach the most appropriate outcome for their care need.

This is a busy role, in a team of 16. There are excellent opportunities to progress with the company. The role is based on 4 sites around Bracknell, so ideally you will be a driver.

Key Responsibilities

  • Answering all incoming patient calls
  • Using integrated systems to-
    • Book appointments with the appropriate clinician
    • Signpost patients to alternative services or information sources
    • Dealing with patient queries, primarily over the phone
  • Administration duties and data entry
  • Rota'd time of front desk at each site dealing with walk in patient appointment requests/other queries and/or requests

Key Requirements

  • Previous primary care experience is desirable however experience within a customer service or call centre environment is also suitable
  • Good communication skills
  • Good customer service skills with a polite and personable manner
  • Strong organisational skills with an ability to follow procedures and multitask
