As a Community Nurse, the responsibilities include:

  • Providing high standards of patient care in partnership with healthcare professionals
  • Teaching patients/carers to self-administer therapy or administering therapies to patients according to service-specific protocols
  • Participating in telephone calls, virtual calls and home visits as part of Patient enablement Programmes
  • Assisting in providing a solution that supports and assists the patient and the healthcare professional in delivering a seamless service between hospital and home
