Our Client, Bury Council, is seeking an Adults Social Worker to join their Hospital Discharge Team.

Responsibilities of this role:

  • Working with Vulnerable Adults within a hospital setting
  • Report writing and making assessments
  • Developing care plans
  • Care management
  • Managing complex cases
  • Liaising with other professional bodies
  • Arranging safe and timely discharges for service users
  • Self-directed support / personal budgets

Requirements of this role:

  • Must be a Qualified Social Worker, registered with Social Work England.
  • Must has at least 3 Years experience.
