Somerset Council are looking for an Intermediate Care Social Worker to join their team. This role would entail working with members of the community to perform risk assessments to protect their safety. The right candidate will be skilled in communicating and connecting with people refusing services who are at risk.
Key responsibilities include:
- Crisis resolution
- Care plan formulation
- Prepare and present reports
- Facilitate the provision of a needs-led service
- Establish professional relationships with service users, carers and their families
- Contribute to multi-agency team working
Requirements for this role are:
- A professional qualification in Social Work
- At least 2 years of post-qualifying experience
- A SWE registration
