Intermediate Care Social Worker
London Borough of Lambeth are looking to expand their current team with a dedicated Intermediate Care Social Worker looking for an opportunity to join a well-structured and organised team. As well as joining a supportive team our client also offers a competitive salary, opportunities for training to enhance your skills and potential progression within your team. This Intermediate Care Social Worker role will be for an initial 3-6 months with potential for extension subject to internal circumstance and your performance.
Intermediate Care Social Worker responsibilities include:
- Intermediate Care is looking for three experienced Qualified Social Workers to join their team which is an integrated service with Adult Social Care and NHS services for adults and older people.
- Intermediate Care provide Urgent Response service to prevent service user's from being admitted into hospital and provide Rehab and Reablement to service users in the community
- Candidates must be exceptional in delivery of services to service users
Minimum requirements / Experience preferred:
- Post qualifying experience of working as an Intermediate Care Social Worker or Adults and Older Persons Physical Disability experience, Hospital discharge experience, Community Neuro or other multidisciplinary team working experience.
- Have excellent working knowledge of the Care Act, Mental Capacity Act, safeguarding and other complex requirements.
- Candidates must have the ability to work at a fast pace under pressure
- A good working knowledge of MOSAIC would be an advantage.
Minimum Requirements:
- SWE Registration
- 1 year post qualifying experience within the UK Social Care industry
- Enhanced DBS/CRB (we can obtain for you)
- Eligibility to work in the UK
