We are currently seeking an experienced and motivated Senior Social Worker to join our team in Hillingdon. As a Senior Mental Health Hospital Discharge Social Worker, you will play a key leadership role in facilitating the safe and effective discharge of patients from our hospital. You will utilise your expertise to guide and support the team in conducting assessments, developing discharge plans, and coordinating post-discharge support.

Key Responsibilities:

  • Providing leadership and supervision to a team of social workers involved in the discharge process.
  • Conducting complex Care Act assessments to determine the support needs of individuals being discharged from the hospital.
  • Undertaking capacity assessments to assess decision-making ability regarding care and treatment.
  • Leading Section 42 (s.42) enquiries to safeguard vulnerable adults at risk of harm or neglect.
  • Collaborating with multidisciplinary teams, external agencies, and stakeholders to develop comprehensive discharge plans.
  • Providing expert advice and guidance to team members on best practices in mental health discharge planning.
  • Ensuring compliance with relevant legislation, policies, and procedures related to mental health care and discharge planning.
  • Commitment to upholding the rights, dignity, and well-being of individuals with mental health needs.
  • Work collaboratively with a range of stakeholders to achieve positive outcomes for service users.

Requirements:

  • Qualified and registered Social Worker with significant experience in mental health settings.
  • Thorough understanding and extensive experience of working within the Care Act 2014.
  • Proven track record of conducting complex Care Act assessments, capacity assessments, and s.42 enquiries.
  • Strong leadership and supervisory skills, with the ability to motivate and support a team.
  • Excellent communication and interpersonal skills, with the ability to engage effectively with service users, families, and colleagues.
