DEBRA have an exciting opportunity, not to be missed! We are currently looking for an Area Community Support Manager (North) to join our friendly team. Working from home full time 35 hours per week with attendance to the DEBRA Head Office in Bracknell, Berkshire as required for training/team meetings. Ability to travel to client home visits and clinics throughout the Midlands and North of UK is essential. Areas include Manchester, Leeds, Sheffield, Liverpool, Newcastle. The Role

Reporting to the Community Support Team Leader, the purpose of this role isto provide a community support and advocacy service to people whose lives are affected by Epidermolysis Bullosa (EB). Through effective communication and liaison with all sections of the community and other stakeholders, you will be at the forefront, visiting families in their home. The ideal candidate must be self-sufficient and seek support from their line manager when needed and work towards our aim, which is to make the lives of the EB community as best as it can be.

Key Responsibilities

Travel to home visits within your region to support our members with a range of different advocacy support needs. Visits are ‘as and when required’ but on an average month, Community Support Managers attend up to 4 home visits.

Case management – provide information / resources and connect with members within agreed timescale.

Ensure case work is reported and logged on our internal CRM.

Attend EB clinics within your region, with an aim to promote our service, and support our members and NHS colleagues. Clinic visits are shared on a rota basis, with a minimum of 1 visit every month.

Build on knowledge and expertise in your chosen ‘Area of Specialist Interest’. Areas of interest may include education, benefits, workplace, bereavement, housing.
