Job Title: Adult Hospital Social WorkerHourly Rate: £20-£26 (dependent on experience)Location: NewcastleJob Summary:We are seeking a highly motivated and experienced Adult Social Worker to join our dedicated hospital team. You will play a vital role in providing high-quality social work services to adult patients, working collaboratively with a multidisciplinary team to assess needs, develop care plans, and support patients through their hospital journey and beyond.Key Responsibilities:* Conduct comprehensive assessments of adult patients.* Develop and implement person-centred care plans, considering physical, emotional, and social needs.* Collaborate effectively with medical staff, nurses, occupational therapists, and other professionals.* Advocate for patient's rights and ensure their voices are heard.* Liaise with families, carers, and other relevant agencies.* Facilitate timely and appropriate hospital discharges.* Support patients with their transition back to the community, linking them with necessary services.* Contribute to the ongoing development and improvement of the team's services.* Comply with all relevant legislation and professional guidelines.* Maintain accurate and up-to-date case records.Essential Criteria:* A recognised Social Work qualification* Current registration with the Health and Care Professions Council* Significant post-qualifying experience in adult social work.* Experience working within a hospital setting is highly desirable.* Excellent communication, assessment, and care planning skills.* Ability to work independently and as part of a multi-disciplinary team.Desirable Criteria:* Experience working with specific patient populations (e.g., older adults, people with mental health conditions).* Knowledge of relevant legislation and policy frameworks.* Experience with case management software.Benefits:* Competitive hourly rate of £20-£26.Apply
