Key Responsibilities
- Conduct assessments of vulnerable adults to determine their care and support needs.
- Develop and implement care plans, working collaboratively with service users, families, and multidisciplinary teams.
- Provide ongoing support and advocacy to individuals, helping them achieve their maximum potential for independence and well-being.
- Participate in case conferences, reviews, and other meetings as required.
- Maintain accurate and up-to-date records, ensuring compliance with legal and regulatory standards.
- Work in partnership with community organisations and agencies to provide comprehensive services.
- Contribute to the prevention and management of safeguarding concerns, ensuring the safety and welfare of vulnerable adults.
- Keep current with changes in legislation, best practices, and social work methodologies through continuous professional development.
Qualifications and Skills
- Degree in Social Work and registration with Social Work England.
- Prior experience in adult safeguarding and social work practice.
- Strong interpersonal and communication skills.
- Ability to work effectively as part of a team and independently.
- A compassionate and empathetic approach to client care.
- Knowledge of relevant legislation and policies.
- Commitment to ongoing professional development.
