Social Worker - Assessment and Intervention TeamLocation: OldhamJob Type: Full-time

We are seeking a dedicated Social Worker to join our Assessment and Intervention Team. The successful candidate will be responsible for managing a caseload, undertaking assessments, developing Care & Support Plans, and revising them as needed. This role is crucial in improving life opportunities for Service Users, Carers, and Families using social work methods and models. The Social Worker will also be expected to develop professionally and promote good practice within the field.

Day to Day of the Role:Prepare for social work contact and involvement, including case review and information evaluation.Undertake assessments of support needs and assist in screening referrals.Develop, deliver, evaluate, and update Care & Support Plans to resolve social care, health, and well-being difficulties.Work with Service Users, Carers, and Families to make informed choices and decisions.Be proactive with respect to Safeguarding issues, following relevant policies and procedures.Identify, assess, and manage risks, contributing towards positive behavioural change to reduce risk.Maintain high data quality on social care systems and ensure confidentiality.Prepare for and participate in meetings and decision-making forums, including Court appearances and Case Conferences.Advocate on behalf of Service Users within the boundaries of corporate and directorate policies.Develop and maintain working relationships with partnership agencies.Embrace changes and advances in working practices, particularly those involving ICT.Participate in formal and reflective supervision sessions and professional development activities.

Required Skills & Qualifications:A Social Work degree or equivalent recognised qualification, e.g., DipSW.Registration with HCPC.Experience in undertaking assessments and producing Care & Support Plans.Ability to prioritise workload and maintain accurate case records.Experience working in a multi-disciplinary team.Analytical skills for collating information and report writing.Negotiation skills and interpersonal skills to deal with conflict.Proficiency in IT, including word processing, spreadsheets, and databases.Detailed knowledge of relevant legislation and guidelines in social work.Understanding of equalities and diversity issues.Willingness to undertake additional qualifications and training.Ability to travel independently across the Borough of Oldham and work unsocial hours if necessary.
