Job Description: Social Worker (Community Team) - North Yorkshire

Location: North Yorkshire

Rate: £32 per hour

Position Overview: The Social Worker in the Community Team for North Yorkshire will play a crucial role in providing comprehensive social work services to individuals, families, and communities within North Yorkshire. This role requires a deep understanding of social issues, strong interpersonal skills, and a commitment to improving the lives of vulnerable individuals.

Key Responsibilities:

  1. Assessment and Intervention:

    • Conduct comprehensive assessments of individuals and families to identify needs, risks, and strengths.
    • Develop and implement care plans tailored to the unique needs of each individual or family.
    • Provide timely and effective interventions to address safeguarding concerns and support positive outcomes.
  2. Case Management:

    • Manage a caseload of complex cases, ensuring timely and appropriate interventions.
    • Work collaboratively with multidisciplinary teams, including health professionals, educators, and community organizations, to ensure coordinated care.
  3. Advocacy and Support:

    • Advocate for the rights and interests of service users, ensuring they have access to necessary services and support.
    • Provide emotional support, guidance, and practical assistance to individuals and families facing challenges.
  4. Collaboration and Partnership:

    • Collaborate with other agencies and organizations within North Yorkshire to promote integrated service delivery.
    • Participate in team meetings, case conferences, and training sessions to enhance professional development and knowledge sharing.
  5. Documentation and Reporting:

    • Maintain accurate and up-to-date records of assessments, interventions, and case progress.
    • Prepare comprehensive reports and documentation in accordance with organizational and regulatory requirements.
  6. Continuous Improvement:

    • Stay informed about best practices, legislation, and policies relevant to social work practice in North Yorkshire.
    • Engage in continuous professional development and participate in supervision and reflective practice sessions.

Qualifications and Experience:

  • Qualified Social Worker with relevant professional registration (e.g., HCPC).
  • Proven experience in social work practice, preferably within a community setting.
  • Strong knowledge of social work principles, legislation, and policies.
  • Excellent communication, interpersonal, and organizational skills.
  • Ability to work effectively within a multidisciplinary team and adapt to changing situations.
