Job description:

A registered Occupational therapist is required to assess and treat patients requiring ward-based rehabilitation and assessment for discharge planning.
Experience of community-based pathways would also be desirable.
Supporting patients in their treatment, recovery and reablement with the clinical pathways including Frailty , Stroke and Orthopaedics.
