As our Lead Supported Living Worker, you?ll be at the heart of supporting our customers who have complex mental health needs to develop their skills to live more independently in our registered CQC service. Typical day as a Lead Supported Living Worker

  • Creating support plans with your customers and coordinating our small team of Support Workers working to the plans.
  • Carrying out risk assessments, support planning, goal setting and regular reviews.
  • Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.
  • Supporting customers with their personal care and medication needs outlined in their support plan
  • Support to attend appointments, take part in hobbies and interests or attend college or work
  • Cuppas, chats and catch ups.There?s no rushing from one customer to the next here! Plenty of time to collaborate with colleagues too.

You bring

  • You?ll have or be willing to work towards your Level 3 Health and Social Care Certificate
  • Passion to support our customers to live their best life, working collaboratively with an eye for detail.
  • Experience of creating person-centred support plans and supporting colleagues in working to work to the plans.
  • Experience of coordinating and assessing customer referrals.
  • Able to work using initiative, remaining calm under pressure and have a resilient approach.

Our manager is Paula, she has been with Homegroup for 18 years starting as Support Coordinator with progression and coaching into a Registered Managers role. Thereare10of us in the team and we don?t just love to support our customers, we?re there for each other too!

To be successful in our roles we need plenty of cake, we enjoy celebrating with our special vintage afternoon creams teas ,Lianne loves to bake, Paula enjoys showcasing her tea sets, Louise loves making and drinking tea.

The team are connoisseur of cream teas and must have ingredient is sharing strawberries and jam, as a team we all encourageand inspire each othertoenhance all our creative ideas.

Job details

  • Where we?re flexible with you, we do need to meet the needs of our customers. That may mean adapting your start or finishing times on occasion.
  • We provide 24-hour support to our customers each day and every day You?ll work on a rota basis between the hours of 8am and 9 pm and take your turn working days, evenings, weekends and bank holidays and will also be required to complete sleep-in shifts.
  • We need you to be flexible to meet the needs of our customers, but we aim to give you the time you need to spend time doing the things that matter to you!
  • Able to use technology for creating and updating support plans, complete online learning and to collaborate with colleagues.
  • You?ll need an Enhanced DBS check done and we pay for that.
