Key Responsibilities:
- Work closely with young adults leaving care, assessing their needs and aspirations.
- Develop and implement comprehensive pathway plans, promoting independence and well-being.
- Provide practical support, advice, and advocacy to care leavers on education, employment, housing, and health matters.
- Collaborate with external agencies to create a network of support for care leavers.
- Maintain accurate and up-to-date records, ensuring compliance with statutory requirements.
About You:
- Qualified Social Worker registered with Social Work England.
- Experience in working with care leavers, understanding the challenges they face.
