Role: Mental Health Practitioner

Medical Staffingis delighted to offer a great opportunity for a highly motivated and experienced Mental Health Practitioner who wishes to develop their career within an exceptional primary care environment.
The dutiesof the role may vary from time to time depending on current and evolving workloads and staffing levels:
- You will be involved in providing a high level of safe and effective care.
- To work independently in the Surgery and the Community, providing assessment, treatment and referral of patients with specified mental health problems and unexplained symptoms.
- Keep detailed records of all clinical activity in line with service protocols on the GP record systems.
- Review medication for therapeutic effectiveness appropriate to patient needs and in according with evidence based practice and national and practice protocols.
- Adhere to all Trust policies, procedures and guidelines.
- Be responsible for the assessment, planning, implementation and evaluation of care.
- To liaise with and refer onto secondary mental health services, voluntary and public sectors agencies and community support groups.
Key requirements:
- Excellent communication skills, both verbal and written
- Ability to establish rapport and build therapeutic relationships with patients
- Strong organizational skills and attention to detail
- Ability to follow care plans and treatment protocols
- Demonstrated leadership abilities in a team setting
- Proficient in using computer systems and software for documentation purposes
