Our Client, the Yorkshire Collaboration, is seeking an Adults Hospital Social Worker to join their Team.

Responsibilities of this role:
- You will manage a varied caseload, which will include capacity assessments, best interest decision making, Care Act assessments, and being part of a duty rota.
- You will deliver exceptional social care to the adults we are supporting and their carers and promote choice and control as much as possible.
- The team is looking to appoint an individual who is enthusiastic to engage in adopting strengths based, person centred assessments.
- You will engage in outcomes focused and preventative conversations which build on the strengths of people and their local networks.
- You will predominantly be supporting with hospital discharges at either our sites at Pinderfields or Dewsbury District Hospitals.
Requirements of this role:
- Must be a Qualified Social Worker, registered with Social Work England.
- Must have at least 2 years post qualifying experience
