Population Health RN Care Coordinator
Job Summary:
The RN Care Coordinator is responsible for delivering and overseeing care management services for medically and/or socioeconomically complex patients. This role involves collaborating with clients and multi-disciplinary teams to develop and implement patient-centered strategies that support health-related goals. The Coordinator will collect and analyze patient-level data, assist in care plan development, and serve as a mentor to staff on best practices in care coordination and motivational interviewing.
This position is 100% on-site. Must be able to commute.
Education and/or Experience:
- Graduate from an Accredited School of Nursing; Current Maryland Nursing License.
- At least five years of nursing experience in community-based, home-based, or ambulatory care settings.
- Chronic disease management experience.
- Encouraged to maintain membership in professional organizations.
Major Duties and Responsibilities:
- Analytical and Critical Thinking:
 - Develop systems to engage patients in self-management and care navigation.
 - Ensure appropriate community resources and ancillary services are in place.
 - Identify high-risk patients, provide additional outreach, and follow up regularly.
 - Provide complex case management, chronic disease management, and medication management.
- Teamwork:
 - Role model and mentor RN Care Managers on assessing and addressing patient needs.
- Communication:
 - Deliver interventions demonstrating sensitivity to cultural diversity and patient backgrounds.
 - Facilitate disease prevention and health promotion with nursing staff, patients, and families.
- Patient Focus:
 - Provide education, information, and support related to care goals.
 - Act as a patient advocate, addressing barriers to care or compliance.
- Checking, Examining, and Recording:
 - Maintain accurate patient records and confidentiality.
- Willingness to Learn:
 - Engage in professional development activities to stay updated on care management practices.
 - Attend training sessions and staff meetings as assigned.
Skills and Abilities:
- Prioritize competing responsibilities and manage complex caseloads.
- Motivational interviewing skills.
- Supportive coaching and mentorship to colleagues.
- Experience with Electronic Medical Record documentation.
- CPR certified.
- Flexibility and adaptability to a changing environment.
