Unit Description: The Intake and Referral department is at the core of our home health and hospice operations, bridging referring providers, program leaders, clinicians and a patient/family's need for timely information about the next steps in their care. Our Intake and Referral team, comprised of RNs, hospital liaisons/complex care coordinators, and team assistants, is the first point of contact for a successful referral that effectively transitions a patient to home health and hospice services. Through a blended role that balances an onsite presence at our main referring hospital (UVM Medical Center) with a hybrid opportunity, the Intake & Referral team’s Intake Nurse and Hospital Liaison supports patients and referring providers by providing education and information about the services we offer, attending rounds and coordinating patient discharges, reviewing extensive clinical documentation and determining patient clinical eligibility for services, writing orders for care and providing general information for patients, families, providers and community members.

The Registered Nurse – Intake & Hospital Liaison position requires an Associate's degree in nursing, RN license, and at least two years of RN experience.
On-call: Not required
Incentives: $10,000 bonus for external applicants
