UP Health System ā€“ Portage in Hancock, Michigan invites qualified Advanced Practice Providers to join our Internal Medicine clinic team. Nurse Practitioners and Physician Assistants to be considered.

Essential Functions:
ā€¢ Ā  Ā Provide exemplary customer service to patients, visitors and coworkers.
ā€¢ Ā  Ā Provide convenient, friendly patient care to all who come through our door seeking health related services.
ā€¢ Ā  Ā Obtains patient history and performs assessment via observation, interview and examination.
ā€¢ Ā  Ā Provide direct treatment and management of health conditions, including use of prescriptive authority and referral to other healthcare providers or community resources.
ā€¢ Ā  Ā Documents patient care performed, assessments made, and orders given.
ā€¢ Ā  Ā Oversees, directs and supports on site laboratory, x-rays, injections, and collections for direct access services, if applicable.
ā€¢ Ā  Ā Provide counseling and education of patients and their families/caregivers concerning preventative health, treatment options and community resources.
ā€¢ Ā  Ā Provide care in the context of promoting age- and culture-appropriate informed choices, shared decision making, and the right to self-determination.
ā€¢ Ā  Ā Working environment subject to varying fast paced conditions during peak seasons.
ā€¢ Ā  Ā Maintains clinical core competency.


Position Requirements:
Masterā€™s degree required
Current MI Nurse Practitioner or Physician Assistant licensure required
Annual influenza vaccination or declination required

Position Details:
1.0 FTE (full-time, 40 hours per week)
Day shift, weekdays only
8 hour shift
On-call rotation
