The position characteristics reflect the most important duties, responsibilities and competencies considered necessary to perform the essential functions of the job in a fully competent manner. This position is responsible for the coordination of the clinical activities within the organization. Utilizes skills in organization, leadership, planning, implementing, evaluating and providing care as it relates to the age and specific needs of the patient.

ESSENTIAL JOB FUNCTIONS:

• The RN Chest Pain & Stroke Coordinator has regular internal contacts, which will involve hospital ancillary services (e.g. nursing, laboratory, radiology, purchasing, engineering, housekeeping, pharmacy) as well as contact with Physicians in the performance of duties and, if applicable, the RN Coordinator has regular external contacts which will involve (e.g. Home health, nursing homes, durable medical equipment, ambulance/transportation services, and product vendors).

• Organizes and prioritizes clinical responsibilities, collaborates with physicians on patient care issues and treatment modalities. Collaborates with other health care providers and acts as a patient advocate.

• Implements and coordinates in collaboration with the Medical Director, Program Director, Chest Pain Committee, Stroke Committee, Quality Department, and other resources to ensure quality services and outcomes; integrates the Quality Assurance Program with the hospital. Ensures database accuracy and utilizes database reports for Quality Assurance Program.

•Meets quarterly with Chest Pain Medical Director and Chief Medical Officer to review chest pain data metrics and outlier reports.

• Analyzes and evaluates information needs and trends regarding chest pain/AMI/Acute Coronary Syndrome (ACS) patients, patients with signs and/or symptoms of a Stroke or TIA,  equipment, staff education and training toward the goals of quality; collaborates with Quality Director Medical Director to formulate corrective action plans.

• Assists with organization of clinical staff educational functions, activities, and case management development by following clinical practice guidelines.

• Initiates and participates as a change agent to improve work environment, work process, or patient care including coaching.

• Obtains, by self-initiative, advanced knowledge and credentials, and serves as a role model/educator; implements standards of care and adheres to policy and procedures and all regulatory requirements (e.g. The Joint Commission, Medicare, and FDA).

• May assist or participate in providing community education.

• Services as Chest Pain Center Coordinator and Stroke Coordinator and supports initiatives to maintain certification standards .

 

 

Works within the framework of the State Nurse Practice Act and follows all department and the hospital policies and procedures. Complies with hospital and nursing standards of practice in the performance of job duties and responsibilities.


Qualifications

Education:

• Graduate of a school of Nursing, required.

• Bachelor’s degree from an accredited university, preferred.


Licenses:

• Current Registered Nurse licensure, required.


Certifications:

• Certified Basic Life Support (BLS) required.

·       Advanced Cardiac Life Support (ACLS) desirable.

•Certification Cardiac Vascular Nursing (CCVN) or Certification in Emergency Nursing (CEN) or Certification in Critical Care Nursing (CCRN) desirable.

 

Experience:

• Minimum five (5) years’ clinical experience; critical care and/or Emergency Department experience required.

• Minimum of two (2) years of prior supervisory or health care management experience required.
