Job Summary

The Bariatric coordinator's role is to navigate weight loss patients through the continuum of care. Serve as a concierge service to improve better customer outcomes and facilitate optimal transition of care.

The bariatric coordinator provides individualized assistance to patients and their families in overcoming barriers to care, facilitating timely access to care, and providing education and support. The patient's trajectory through perioperative phases and transition will be seamless and coordinated in collaboration with the multidisciplinary team. Provides maintenance of patient tracking system, coordinates appointment scheduling among disciplines, identifies and troubleshoots barriers to follow-up care, and connects patient financial counseling advocates if needed.

Essential Functions

  • Strong organizational skills
  • Ability to ensure adherence to referral management protocols, policies, and procedures
  • Ability to build and support positive, effective relationships across the continuum and with the patients and communities we serve.
  • Ability to effectively communicate with other medical providers and their offices, patients, and families in order to promote seamless coordination and scheduling
  • Communicates consistently with providers ensuring a seamless plan of care as well as follow-up care. Responsibilities
  • Patient assessment and identification of patient-specific needs, identifying barriers and having the knowledge base to overcome these barriers, locating resources, coordinating services, providing education on treatment side effects, and developing structured means for patients to report.
  • Follow patients through the continuum of care, including inpatient admissions, and collaborate with inpatient care management resources.
  • Serves as primary contact and advocate for the patient; builds and maintains the trust of the patient and family, provides emotional support, navigates the healthcare system using knowledge of the intricacies of each system and serves as the patient advocate assuring timely access to care.
  • Serves as a point of contact for providers.
  • Collaborates with the multidisciplinary team creating open communication with the treating physician, the primary care physician, and their support staff.
  • Establish a referral pathway thereby decreasing delays in treatment by working collaboratively with the medical/surgical weight loss team and independent surgeons to maintain a network of physician support of best practices and the delivery of care.
  • Must serve as an active member of the North Carolina Bariatric Service Line committee. Work well with team and others to collect data, track outcomes, and support the strategic planning process.
  • Maintains accurate and complete documentation and ongoing assessments of the role in decreasing disparities in care.
  • Collaborates with the multidisciplinary team to coordinate patient care.
  • Educates patients and their caregivers on the disease process, treatment, and resources.
  • Coordinates referrals both with Frye and within the region.
  • Provides emotional support related to the clinical condition.
  • Advocates for patients and caregivers for appropriate utilization of resources.
  • Implements and measures quality improvement initiatives.

Certifications: Basic Life Support (BLS)

Minimum Education: High School Diploma

Qualifications: Minimum of 3 years of patient relations in healthcare
