Position Summary:

The RN Case Manager collaborates with the patient, family, and interdisciplinary team to facilitate the patient’s progress through the acute episode of care. The primary focus on patient transition to the appropriate level of care while facilitating clinical appropriateness and LOS.  Promotes effective utilization and monitoring of health services, collaborates, and communicates with the healthcare team and patient/caregiver to manage care and transitions. Works to appropriately apply benefits and utilization management serving as a resource to the patient or member and healthcare team.

Minimum Qualifications:

Required:  

·         Graduate of a School of Nursing, College or University

·         Current PA RN license that is active and in good standing

·         3 years experience of acute care as a Professional Nurse or RN experience with significant acute care hospital Case Management experience.

Preferred:

·         Bachelors Degree in Nursing

·         Previous Case Management experience

·         Case Management certification preferred

·         Case Management Certification, CP
