Position Summary: 

The Care Coordination & Transitions Specialist (CCTS) works collaboratively with other leaders across the health system to develop and implement processes that contribute to the “Triple Aim” of health care improvement: improving the experience of care, improving the health of the population, and reducing per capita cost. This is accomplished by providing coordination between outpatient and inpatient care management teams, physicians, advanced practice providers and post-acute care.

·         Required: degree in Nursing (RN)  

·         minimum of five years of experience in the healthcare setting.

·         Current nursing license in the state of Pennsylvania
Ability to work as a part of a multidisciplinary team in a professional manner.

·         Ability to adapt to evolving goals and requirements and to test new approaches to improve patient care.

·         Displays critical thinking and problem-solving skills.

·         Displays evidence of excellent verbal, written, and interpersonal skills. Skilled in developing collaborative relationships with internal and external stakeholders.

·         Ability to use independent clinical judgment and compassion when carrying out tasks.

·         Excellent organizational skills.

·         Computer skills with proficiency in Microsoft Office & Outlook.
