Purpose & Impact:

The purpose of the Program NurseĀ is to provide Health Services to children and families. Assists in the identification, coordination, completion of service and evaluation of health or family problems of enrolled children. Maintains awareness of current health services available in the community and works with physicians and other local health care providers to meet the needs of enrolled children. Responsible for the coordination of health follow-up services to identified children. Documents all health/family services that have been provided. Monitors health records at Head Start and Early Head Start center sites. If applicable to the Program, this includes the Early Head Start-Child Care Partnerships Expansion grant. Participates in the HS/EHS Quality Assurance efforts.Ā Ā 

Essential Functions:Ā 

  • Reviews childrenā€™s files during the enrollment process to identify any health concerns and assess the programā€™s ability to meet a childā€™s individual health needs before admitting a child into the program.
  • Evaluate, screen, and approve children with identified health needs.
  • Conduct child staffing meeting to address identified health concerns, accommodations required, development of health plan, and/or assess childā€™s appropriateness for admission based on health needs.
  • Create and write an Individual Health Plan for children with identified health, nutrition, or disability concerns based on physician orders in collaboration with Child Health and Safety Manager.
  • Assist in administering lead screenings.
  • Monitor and track medication administration and health documentation for individual children with a health plan and/or identified health concern.
  • Work in collaboration with the Disabilities Coordinator, Health Coordinator, and FACE Specialists regarding an individual childā€™s health needs.
  • Conduct regular site visits for the center with children that have identified health plans as well as to conduct medication management review.
  • Engage parents regarding their childā€™s health needs to ensure resources are provided and appropriate accommodations are in place.
  • Maintain efficient documentation and updated health case notes in Child Plus database.
  • Conduct home visits as needed to educate, consult, and provide information to parents.
  • Obtain parent permission for further information from other health providers if needed to develop a childā€™s individual health plan.
  • Maintains a good working relationship with physicians, dentists and other local health care providers in order to provide optimal healthcare for enrolled children.
  • Assist families with referrals and resources to appropriate community services for identified health needs.
  • Coordinate and 7 assist parents with scheduling for their childā€™s follow-up treatment.
  • Administers first-aid or other emergency care as necessary when on site.
  • Review and updates child medical records for completion, follow-up and/or current compliance every 30 days.
  • Maintains strict confidentiality at all times, including the ability to recognize situations in which confidentiality is required.
  • Attends all scheduled workshops and meetings.
  • Attends all required staff and parent meetings and activities as deemed necessary.
  • Prepares and delivers health-related training and technical assistance (medication administration, medical procedures, health, and wellness, etc.).
  • Other related duties as assigned.Ā Ā 

QualificationsĀ 

Education: Registered Nurse. BSN or Higher or related degree preferred.Ā Ā 

Experience: Requires two years of experience in community health, pediatrics or school nursing. Bilingual preferred: English/Spanish or English and other languages present in the local area.Ā 

Also required:Ā  Certification in First Aid and CPRĀ within 90 days from the dateĀ of hireĀ and successful completion of a Level II background screening, local criminal record check, health, TB and drug screening prior to hiring. Bilingual preferred: English/Spanish or English and other languages present in the local area.Ā Ā 

Knowledge, Skills & Abilities:

  • Clinical experience conducting basic developmental screenings such as vision, hearing, height/weight, lead, hemoglobin/hematocrit for children: infants to age 5.
  • Knowledge of human development and health.
  • Knowledge of social services and health services available in the area.
  • Knowledge of computer data entry processes.
  • Knowledge of family-centered practices.
  • Ability to communicate ideas and instructions orally and in writing.
  • Ability to write reports and business correspondence.
  • Ability to speak before groups of individuals, such as Committees and Staff meetings.Ā Ā Ā
