Associate Director of Integrated Care                

Inspīr Senior Living, Maplewood Senior Living’s new brand of world-class, urban senior living properties, is now hiring Home Health Aides (HHA) | Certified Nursing Assistant (CNA) for its new Inspīr Carnegie Hill property. Inspīr Carnegie Hill is located on the Upper East Side and stands 23 stories with 215 residential units. Typical Work Duties & Functions

  • Develop and lead clinical training programs to ensure staff are highly trained and are able to demonstrate proven competencies.
  • Foster a learning culture and participate in orientation and training initiatives.
  • Assesses resident, family and staff needs and develops strategic and operational plans for the provision of resident care and the delivery of all clinical services and programs
  • Assists in overseeing all aspects of resident care and clinical services, nursing care, ancillary services, provider partners and care-related aspects of quality assurance, regulatory compliance, safety and risk management and resident/family/staff satisfaction
  • Assists in leading Inspīr health, wellness and care teams and supervises Section Directors of Resident Care to promote best practices and ensure optimal delivery of care and clinical services to all residents
  • Facilitates and follows evidence-based Standards and Practices, performance standards, and standardized workflows to ensure optimal efficacy, efficiency and outcomes
  • Collaborates with the Senior Director of Integrated Care to develop strategy, business plans and budgets that focus on all aspects of clinical and facility operations including setting priorities, staffing models, reducing costs, and maximizing revenue while ensuring outstanding care and services to residents
  • Works collaboratively with other Department Heads to develop priorities and goals to address resident needs throughout the community
  • Plays a key role in addressing and resolving policy or regulatory infractions or violations identified by staff or state regulators including developing and implementing action plans, corrective measures and new policies or Standards and Practices
  • Uses Key Performance Indicators and business analytics to set goals, drive strategy, inform planning and monitor performance of Nursing Department
  • Performs regular audits to ensure that each clinical department is meeting all Inspīr and state standards and regulatory requirements
  • Must adhere to all applicable compliance requirements and Inspīr safety policies and procedures including but not limited to: understanding and mastering state regulations and ensuring compliance at all times; completing required compliance training in a timely manner; reviewing Code of Conduct at least annually and promoting it in the workplace; Promoting the Inspīr compliance program; adhering to Inspīr’s Policy and Procedures; reporting, promptly and in good faith, any instances of suspected fraud, waste and abuse,  suspected privacy and/or security incidents, or any other compliance concerns; and ensuring confidentiality of resident PHI and company proprietary information 

Minimum Qualifications

  • Minimum of Bachelor-level NY-state licensed Registered Nurse (RN) in good standing, preferably with advanced degrees/training and certification(s)
  • Minimum of 2 years of leadership experience
  • Experience in senior living, LTC, SNF, and/or home health care
  • Minimum of 2 years working with elder adult population with complex medical needs including dementia
  • Well-versed in staffing and staffing models
  • Free of communicable diseases
  •  Proficient with Microsoft Word and Excel as well as electronic health records

  • Must be able to clearly convey and understand information and ideas in English
  •  Must be able to evaluate and select among alternative courses of action quickly and accurately
  • Must be effective in handling problems in the workplace, including anticipating, preventing, identifying, and solving problems as necessary
  •  Ability to assume the duties of Sr. Director of Integrated Care in their absence
  • Must work well in stressful, high pressure situations
  • Must maintain composure and objectivity under pressure
  •  Must have the ability to assimilate complex information and data from multiple sources and consider, adjust, or modify to meet the constraints of the particular need
  • Must be effective at listening to, understanding, and clarifying the concerns and issues raised by residents, guests, or associates
  • Must be able to work with and understand financial information and data, and basic arithmetic functions
  • Ability to stand for long hours and walk long distances without fatigue
  • Ability to bend, stretch, stoop and reach repeatedly without difficulty
  • Exerting up to 60 pounds of force occasionally, and/or up to 20 pounds of force frequently or constantly, to lift, carry, push, pull, or otherwise move objects
  • Ability to work under variable temperatures and noise levels
  • May be required to use equipment such as Hoyer lift
