LSF is looking for a talented Case Manager who wants to make an impact in the lives of others.

Purpose and Impact:

The Case Manage is responsible for the submission of service plans and other assessments required for the compiling of a comprehensive case file toward therapeutic/social services. The Case Manager is required to maintain a flexible, organized, and efficient work schedule and is subject to work extended hours, weekends, and be on-call. 

Essential Functions: 

  • Conduct initial intake interviews and assessments to develop an Individual Self-Sufficiency and Wellness Plan of adolescent, adult, and pediatric ASA Afghan eligible arrivals. 
  • Determine health options available with input from clients, as a 
  • Ensure the timely completion of (assessments) in accordance with LSF, State, and Federal requirements.  
  • Ensure the timely submission of the initial Individual Service Plan after refugee arrival in accordance with LSF, State and Federal requirements. 
  • Document all actions taken and contacts with client in the form of progress notes (efforts) as required by LSF, State and Federal contracts. 
  • Complete and submit relevant information for review. 
  • Provide appropriate referral for additional services as deemed necessary. 
  • Provide and receive regular updates as applicable to client. 
  • Coordinate weekly treatment team meetings with representatives from all relevant departments to monitor progress and ensure highest quality of services. 
  • Maintain physical and web-based client files. 
  • Drive clients to facilitate program services, may include transportation to health clinics, hospitals, or other related appointments as per contractual requirements as needed. 

Other Functions: 

  • Attend all organizational required trainings. 
  • Attend all departmental and program meetings to ensure that up-to-date information is received and/or information of policy changes or practice are adhered to. 
  • Attend training that will enhance professional growth in the area of case manager and documentation or other topics as deemed appropriate by the program director. 
  • Travel as needed for trainings, conferences or to transport youth to destinations located within the U.S. 
  • Maintain a safe, clean and hazard-free work area. 
  • Able to react to change productively and handle other essential tasks as assigned. 

Physical Requirements: 

  • Must be able to drive, have a valid Florida Driver’s License and maintain a clean driving record 
  • Must be able to work flexible and long hours 
  • Must be able to work in a fast-paced environment 

Education: 

  • Bachelor’s degree required in Social Work, Psychology, Human Services, Counseling or other social service field. 

Experience: 

  • 1 year experience preferred working with adolescents, adults, or children in the social services (volunteer and internship experienced included) 

Skills: 

  • Excellent organizational and time management skills 
  • Ability to collaborate with others, including case management staff, refugee services team members, and supervisors 
  • Ability to practice cultural sensitivity and work effectively with diverse populations 
  • Ability to be flexible with working hours and on-call responsibilities 
  • Excellent writing skills 
  • Computer skills sufficient to perform essential functions (using the ORR system, Microsoft Office, and any internal case management software) 
  • Able to speak, read, and write English and a second language fluently according to contractual needs.

  • Must demonstrate sensitivity to our service population’s cultural and socioeconomic characteristics and needs.

  • Reports to Medical Director  
  • Team player with co-workers 
  • Able to work independently and with minimal supervision. 
  • Meeting monthly goal. 
  • Accurate, complete, and timely submission of paperwork. 
  • Adherence to LSF personnel policies and procedures.
