- Completing Care Act Assessments to facilitate smooth transitions for service users leaving the hospital.
- Supporting discharges from intermediate care bed/inpatient rehab settings.
- Collaborating with health colleagues for discharge planning and ensuring the right pathways for individuals.
- Promoting a strengths-based approach.
- Working closely with individuals, families, carers, and community services (reablement, telecare, care homes, domiciliary home care providers) to promote independence and safety post-hospitalization.
- Requirements:
- Relevant Level 3 Qualification in Health and Social Care.
- Experience in producing quality assessments.
- Collaboration with various agencies.
- Understanding of the current Discharge to Assess process.
- Collaborative work with health colleagues in acute and intermediate care settings.
