I have the pleasure of recruiting for Barnsley MBC for a Care Manager/Social Care Practitioner on a temp basis on going

  • Location: Barnsley
  • Responsibilities:
    • Completing Care Act Assessments to facilitate smooth transitions for service users leaving the hospital.
    • Supporting discharges from intermediate care bed/inpatient rehab settings.
    • Collaborating with health colleagues for discharge planning and ensuring the right pathways for individuals.
    • Promoting a strengths-based approach.
    • Working closely with individuals, families, carers, and community services (reablement, telecare, care homes, domiciliary home care providers) to promote independence and safety post-hospitalization.
  • Requirements:
    • Relevant Level 3 Qualification in Health and Social Care.
    • Experience in producing quality assessments.
    • Collaboration with various agencies.
    • Understanding of the current Discharge to Assess process.
    • Collaborative work with health colleagues in acute and intermediate care settings.
